Appointments

For appointments, please call (718) 886-9000 or (201) 886-9000 on weekdays from 8 AM to 6 PM and on Saturdays from 9 AM to 1 PM. Office hours vary from office to office, but we will try our best to provide appointments as soon as possible. Walk-in visits are accepted but may require extended wait time. Please call our operators for more detailed information about each office.

Instructions For Initial Visits

  • Please bring your insurance card, photo ID, and pharmacy information. If your health insurance plan requires a referral to see a specialist, a valid referral must be issued BEFORE the visit. It is your responsibility to obtain the referral.
  • Bring ALL medications you are currently taking with you.
  • All minors are required to be accompanied by a legal guardian. An older sibling or relative who is not a legal guardian is not acceptable. A notarized letter from a legal guardian allowing the accompanying adult to make medical decisions may be acceptable.
  • Please fill out the registration packet before you arrive for your appointment. This will save you time and reduce the waiting time significantly.
